A widespread source for continuous leaps in learning, ImproveCareNow (ICN) is a trailblazing national collaborative of pediatric gastroenterology clinics dedicated to quality improvement. These doctors, nurses, patients, and parents work together to apply comparative evidence methods and ways for patients to take charge of their own health internationally in both rural and urban populations. ICN pilots, manifests, and manages a transformative set of tools and practices that exponentially improve the health of patients and dramatically alters the status quo within and outside of clinical settings, spanning 30 ICN pediatric GI practice sites, 275 physicians reaching 10,000 patients, 100,000 patient visits, with a remission rate of 73 percent.
Wondros, Lybba’s filmmaking partner, depicted the good work that ICN is doing in conjunction with another collaborator Collaborative Care Network, C3N. Together, we’re creating a vastly better care system by harnessing the motivation and collective intelligence of patients and clinicians. Our goal is to connect patients, clinicians and real-time data to improve care across all chronic illnesses, facilitating earlier detection and diagnosis. C3N uses social media for chronic illness care, synthesizes emerging technologies, and builds mentoring networks online and in person.
